The closest things I have discovered to ngrok is something like Packetriot or even Cloudflare Tunnels. There are many ngrok clones out there with varying changes. When I was using Tailscale, I found this interesting blog post of a guy assigning a public DNS to his tailscale ip and did NAT reflection to his local service. Tried it and it worked surprisingly.
